michellemart782 @jaynie911 hello! You take it out of the cover and put it in the microwave if u want it hot. If u want it cold u take it out of the cove and put it in the freezer. I have had one since 2006 and to be honest I use it at least 4 times a month.My daughter has one since 2010 and uses it about 10 times a month because she gets really bad cramps on those days of the b month and she play softball as a catcher so she has plenty of sore days on her back, arms and legs. It’s very useful in general.
