Does this run big or small?

It fits as a medium. I have worn heavy sweaters under it and it worked out fine. It is extremely warm because it goes up so high at the neck.

@allisongre sorry about the time delay. Here I edited and updated the photos. I paired the vest with a black hoodie to keep it simple and showed that is zips up, I also tried to expose the buttons so that you can see they are all there in place, and that the neckline is high as well. This can be worn under a moderately warm coat, or over a very heavy chunky sweater if you are on the west coast. ;)

@allisongre also, keep in mind that I've worn this to brave winters in Northeast China and NYC. Just in case you are in the mid west, east coast, or the south.
