I saw at saks fifth ave that size 46 Italian is size 12. I am sorry that I do not use proper grammar I just had surgery and have a cast on my right arm/hand

Italian 36= USA size 0, 38=2, 40=4, 42=6, 44 =8 & 46=10. But know that these are all on
