uswsim01 @spiritfilled76 Hi - I would say true to size. I am normally a 7-1/2 in most shoes but an 8 in pointed toes, and these fit me perfect. I would say they are comfortable for pointed toe shoes as the toe box is generous. There is no real padding as they are leather soles, I would say the comfort is on par to any Marc Fisher, Sam Edelman or Jimmy Choo shoe...
