Madewell Striped Gallerist Dress, fit and flare shape and short sleeves, thin black and white stripes with zipper back, fits like an XS in Madewell sizing, but tag says S. Some of the hem has come undone but is an easy fix. (See last photo.) Otherwise in great condition.
